- Do you have photographs of property damage and/or accident

- Have any statements been given?
- Names of all liability and health insurance carriers,
including policy and claim numbers.
- Has a police investigative report been obtained?
- Name of law enforcement agency
which investigated the accident.
- Were citations issued to any driver?
- Were traffic control devices involved?
- Was road construction a factor?
- Was weather a factor?
- Were alcohol or drugs a contributing factor?
- Previous automobile collisions?
- Previous injuries or medical
